AC Consent Form

All players need to download and print their consent form (sometimes referred to as the Registration Form) and get it signed by a parent or guardian. Once signed, please seal it in an envelope with the £5 assessment fee with the player's name on it.

On arrival, the coach will have a box for you to drop the envelope in. Please do NOT forget the form or payment because without it, you will NOT be allowed to take part.

To download the consent form, the parent/guardian will need to log into the Player Registration System and look for the button marked 'Consent Form' at the bottom of the player's panel.
